Title: ARRA Grant - Cancer Nurse Coordinator/Navigator
Location: MD-Towson-St Joseph Medical Center
The Cancer Nurse Coordinator/Navigator is responsible for providing guidance, support, and education to patients and their families (in area(s) of specified disease specialty) through the diagnostic, treatment and follow-up portions of their care. The nurse collaborates with health care team members to assess, plan, implement, and evaluate patients' care. This position serves as a resource to all clinical disciplines relative to the assigned area(s) of disease specialty.
The Nurse Coordinator/Navigator functions with the goal of providing quality care to assigned cancer patients and their families by providing guidance and education through the diagnostic, treatment, and follow-up phases of the care continuum.
The Nurse Coordinator/Navigator, in collaboration with the multidisciplinary cancer team, strives to assure cohesive and coordinated patient services. The nurse develops, maintains, and coordinates the multidisciplinary care plan for each assigned patient by incorporating education, promoting Cancer Institute, hospital, and community resources, and evaluating patient and family needs along the continuum of care.
Collaborates with the Cancer Institute leadership and assigned physicians to continually monitor the level of professional practice related to cancer care and initiates actions or programs to assure comprehensive patient care, team work and professionalism
Collaborates with the patient, family, ancillary departments, and community resources to achieve the goal of a multidisciplinary health care team who provides care that is well coordinated and consistent across the continuum of care.
Provides education and support to cancer patients diagnosed and/or being treated at the Cancer Institute
Bachelor's degree in Nursing from an accredited college or university.
Master's degree in Nursing or related field is strongly preferred.
One year of related professional nursing work experience in oncology.
Previous oncology navigator work experience is strongly preferred.
Current BLS Certification.
Certification by the Oncology Nursing Certification Corporation (ONCC) as a Oncology Certified Nurse (OCN) required within one year of date of hire.
