The Case Manager is responsible for coordinating health care in a cost effective manner through teamwork. S/he provides professional assessment, planning, implementation and evaluation of care. S/he oversees provision of patients' medical care by coordinating medical services and providing treatments ordered by the physician to assure patients' medical needs are met. S/he plans, organizes and implements social work services to patients and families. In addition, s/he plans for discharge and aftercare and maintains contact with referring agencies, related people and with community resources used for referral.
Two years Med/Surg experience and knowledge of managed care criteria required. Two years case management experience preferred. Computer literate and strong interpersonal skills required. Current NJ RN License required.
Associate's dsegree in Nursing required. Bachelor of Science in Nursing (BSN), CCM is preferred.
