**MEMBERS ONLY**SIGN UP NOW***. is currently searching for afull time, evenings,Case Manager, for the Emergency Department at the University Medical Center of Princeton at Plainsboro. This position is 11:00am-7:30pm M-F.
Position Summary:The Case Manager will ensure seamless coordination of patient care from the point of admission to University Medical Center at Princeton and through the continuum of care across the **MEMBERS ONLY**SIGN UP NOW***.. This position will also ensure the patient safety and satisfaction, optimal and medically appropriate utilization of clinical resources, and appropriate reimbursement from third party payers. The Case Manager will also determine the medical necessity of admission for inpatient care, identify patients, appropriate Observation services, ensure the plan of care meets intensity of service for inpatient care, intervene to avoid delays in plan of care, identify discharge planning needs, and arrange services.
· State of New Jersey Registered Nurse license
· Minimum 3 years in hospital-based Case Management, Discharge Planning or Utilization Review
· Excellent interpersonal and communication skills
· A customer service orientation, teamwork approach, and professionalism
