Medical Case Manager Jobs description

A case manager is a healthcare professional who plans, monitors and evaluates the options and services required to meet a client's health needs in the best possible and cost-effective way. A case manager is generally responsible for the direct care of an assigned patient.

The primary responsibility of a case manager is to enhance the quality of patient care, and promote continuity of care and cost management. A case manager may assist physicians during examination, administer tests and medication and promote effective utilization of clinical resources. A case manager also promotes a patient's self esteem by educating the patient and family about his/her self-care and medication. Partnering with physicians to ensure that all patients receive optimal care in a financially responsible manner also falls under the purview of a case manager's job. For patients whose needs can be met outside a hospital, case managers also facilitate timely discharge to appropriate facility.

To apply for a case manager job, the appropriate minimum degree for preferred licensure or certificate is required. A healthcare professional licensure is preferred as registered nurse, respiratory therapist, physical therapist etc. Other desirable skills are CPR certification, case management credentials, and a high degree of proficiency with clinical documentation. Good communication and interpersonal skills are also important.
