The HYBRID Transplant Liaison serves a key function in supporting transplant equity. The Transplant Liaison provides essential support and management throughout each patient\'s transplant journey, starting from the referral stage up to the completion of the transplant.

A vital link between communities and the healthcare system, the Transplant Liaison advocates for patients and their care partners, fostering a robust connection with the communities they serve. Their role extends to serving as intermediaries between health and social services and the transplant community, aiding in facilitating access and refining the transplant process.

By addressing the Social Determinants of Health (SDOH), Transplant Liaisons aim to simplify the transplant process for underserved patients, reducing health disparities and promoting health equity. This goal is achieved through the establishment of strong partnerships with healthcare providers, local transplant partners, and community resources. Emphasis is placed on confronting socio-economic factors that may cause delays in waitlisting and transplant, contributing to the overall improvement of patient care and outcomes.

This role will travel between offices in the Philadelphia, PA area.\

This is a temporary position for 1 year

ESSENTIAL RESPONSIBILITIES for the Transplant Liaison:\

The primary responsibilities encompass various areas to ensure the best possible outcomes for our patients throughout the transplant process.\

Patient-Centric Communication: The role involves acting as a pivotal communication link between DaVita clinics, transplant centers, the larger healthcare system, and healthcare teams. This ensures that the patient\'s needs are effectively addressed and communicated.\

Care Team Building: A crucial part of the role is facilitating the creation of a robust care team that is dedicated to addressing each patient\'s unique needs during the transplant journey. This ensures a comprehensive and personalized healthcare approach.\

Problem Solving: Leveraging professional expertise to troubleshoot and resolve issues that might arise during the transplant process is a significant aspect of this role, thereby guaranteeing smooth progress to waitlisting.\

Patient Education and Partnership: The central aim of the role is to foster a partnership with patients. This involves educating patients and their care partners on health promotion, transplant procedures, and living donation possibilities. This education-based approach is crucial in facilitating successful waitlisting and transplantation processes.\

Assess, plan, advocate, and offers emotional support for the patient and their care partners in coordination with DaVita clinics, local transplant partners, community organizations, and other relevant parties.\

Connects patients and their care partners with DaVita resources, community resources and social services to address social determinants of health, such as housing, food security, transportation, and financial assistance.\

Provides telephone & In-person consultation and follow-up to support patients and their care partners and dialysis\

Provides comprehensive support to potential living kidney recipients. By facilitating education about the living donor program, coordinating recipient\'s medical evaluations, and ensuring clear communication between the recipient, potential donor, and medical teams. Assisting the recipient in understanding the benefits with receiving a kidney from a living donor.\

Assists in the collection and analysis of data as requested by DaVita, patients and local transplant partners.

Qualifications for the Transplant Liaison:\

Bachelor\'s degree in clinical specialties or health science such as Health Care Administration, Nursing, Social Work, public health, or related health-care field, from an accredited college or university or equivalent combination of experience and education\

Local and familiar with transplant programs: John Hopkins, Henry Ford, University of MD, University MI\

Experience engaging in initiatives that promote health equity and social justice within program communities that improve health outcomes and advocate for underrepresented and vulnerable groups within the community.\

Proficient in health equity and social justice within the communities of the pilot that performed advocacy work, community outreach, policy development, or research eliminating health equity disparities.
