SUMMARY:

Provide patient focused outcome oriented
pharmaceutical care to patients of all ages, including neonatal, infant,
pediatric, adolescent, adult, and geriatric. Facilitate the timely distribution
of drugs, and disseminate drug information to patients and health care
professionals.

 

ESSENTIAL
FUNCTIONS:

Deliver pharmaceutical care to
patients.Promote professionalism and
continually strive to raise the level of professional practice.Interpret physicians' orders as
they relate to the patient's drug therapy.Prepare, compound, and dispense
medications.Enter medication orders onto the
pharmacy computer system and complete other documents related to the dispensing
and control of medications.Evaluate the appropriateness of
medication orders based on clinical data available in the medical record
(including computerized information) and provide recommendations to physicians
when appropriate.Monitor the patient's medication
profile for allergies, contraindications, duplications of therapy, drug
interactions, and potential adverse drug reactions.Contact physicians and nurses to
clarify orders or recommend changes in therapy as appropriate.Respond to drug information
questions.Provide medication teaching to
patients and families on discharge when requested.Monitor adherence to dosing
guidelines and target drug protocols and intervene as necessary.Provide pharmacokinetic dosing.Monitor intravenous nutritional
supplements for patients.After certification and when
consulted, manage intravenous nutritional supplements for patients in area of
responsibility.Maintain patient care plans.Participate in pharmacy quality
management activities.Document therapeutic and cost
saving interventions on the pharmacy PC.Oversee and assume responsibility
of technician activities.Oversee and interpret audits
performed by technicians.Assume responsibility for unit
inspections in assigned areas.Participate in experiential
education of pharmacy students.Check medication orders,
including sterile products, filled by pharmacy technicians.Ensure adequate inventory of
pharmaceuticals and other supplies.Maintain adequate records of
disbursements of controlled substances.Understand and comply with all
state and federal pharmacy laws and regulations.Complete required hospital
education annually.Participate in interdisciplinary
team meetings or rounds in area of responsibility.





















































 

MARGINAL
FUNCTIONS:

Provide educational in-services
to pharmacy and other hospital staff members and to community health
professionals and consumers.Obtain a medication history from
patients.Submit articles for the pharmacy
and hospital newsletters when appropriate.Assist in the development of
MUEs.Serve on committees within the
pharmacy department when appropriate.Represent the pharmacy department
on hospital and medical staff committees as assigned.Review a drug or drug category as
requested by the Pharmacy and Therapeutics Committee.Assist with review and
development of pharmacy policies and procedures.Actively participate in local,
state, or national professional organizations.Serve as preceptor for student
projects.Participate in interdisciplinary
team meetings or rounds in area of responsibility.Provide pharmacy-consulting
services to long term care facilities that have a contract with the pharmacy.Perform DRR on each patient as
scheduled.Understand federal guidelines for
pharmacy for long term care and incorporate them into practice.Review resident care plans.Provide suggestions for and/or
contribute to the LTC newsletter.Pharmacists practicing in a
designated specialty (e.g. Cardiology):Provide formulary reviews as
appropriate.Complete/coordinate MUEs/projects
to evaluate therapy at Mercy
Medical Center
and implement change.Interact with the medical and
nursing staff to problem solve and enhance the care provided to the patients at
Mercy Medical Center.Provide inservices as requested.Contribute to the Pharmacy
newsletter, as appropriate.Assist with the clinical on-call
schedule.Drug Information:Respond verbally and in writing
to questions received in the Drug
Information CenterProvide formulary reviews as
appropriate.Assist with quality assurance
activities of the department as appropriate.Contribute to the Pharmacy
newsletter.IV Room Pharmacists:Responsible for the accuracy and
quality of admixing of IV products prepared in the IV Room.Completes IV Room competences as
required.Ensures that the policies
outlined by the department for USP 797 and other IV Room activities are
followed.Main Pharmacy:Responsible for the timely
delivery of medications from the main pharmacy.Responds to requests from health
care professionals (e.g. drug information, mediation orders) in a timely
manner.Oversees and directs the
activities of pharmacy technicians/interns to ensure pharmaceutical care.Floor pharmacists:Responds to requests from health
care professionals in a timely manner (e.g. drug information, consults,
medication orders).Provide patient education as
requested.Identify home medications as
appropriate.Clinical Staff PharmacistsAssists with the provision of
consults.Assists with the reviews of
profiles for renal dosing IV to PO conversions
and targeted therapies as directed by department.Assists with order entry as
appropriate.Provides patient education as appropriate','!*!MINIMUM
KNOWLEDGE, SKILLS AND ABILITIES REQUIRED:

Pharmacy degree (BS or Pharm.D.)
from an ACPE approved college of pharmacy.Licensed, or eligible for
licensure, pharmacist in Iowa.Previous experience desirable.
