Description
Under the direction and supervision of the Medical Records Supervisor, the registration representative is responsible for assisting with management of the resident care unit and for performing secretarial, clerical and receptionist duties, inventory management of equipment, and service from other departments or outside vendors, inputting and monitor resident charges and record processing.
Principle Accountabilities: % of Time Spent
1.Performs order processing and registration to include: 25%
Pre-registers and Registers patients. Enters Discharges and Transfers into computer system. Completes admission paperwork and assembles charts with all necessary forms. Scans signed information into computer system. Collects orders and checks for any missed orders. Coordinates Tests and procedures with clinical staff.
Call and/or send letters to Physicians requesting necessary H&P and Progress note updates to patient charts. Make sure code status forms are signed and updated. Physician order sheets mailed out monthly/tracked/returned to patient\u2019s medical record for all Private Pay units.
2. Performs division duties to include, but no limited to: 30%
Registers outpatient lab and xray tests in Cerner and completes registration paperwork. Looks up/prints lab and x-ray reports. Copies charts for transfer. Performs computer entry. Checks printers daily for paper, toner and orders repairs or maintenance as needed. Orders office supplies as needed. Assists in maintaining supply processing & dispatch inventory. Carries portable phone. Answers phones with 3 rings and takes messages/transfers calls to appropriate staff member. Faxes reports/orders. Places calls to doctors. Audits charts for appropriate documentation and thins chart monthly or as needed. Notifies RN when Doctor is on division. Reports any repairs or needs to Maintenance Department as needed. Charts/Records information on appropriate forms. Performs Customer Service for patients or visitors and answers questions/directs to appropriate team member. Participates in training of new team members. Attends all required in-services.
3. Medical Record maintenance and processing:20%
Files reports in charts and pulls reports or records for doctors. Sends unsigned orders to Doctors for signature. Check records for completeness and breaks down for transfer to Medical Records within 14 days.
4. Updates Code Status and Patient demographics annually. Keeps current spread sheets of ancillary charges and equipment rentals on Private Pay Wings.
5. Routine Duties include:
Collect and process mail for your assigned unit. Order forms for nursing as needed. Stock cart with forms for Nursing Admission Packets. Assemble Admissions/Transportation/Emergency Packets as needed. Communicate Admissions to Nursing/ Covering Physician and distribute Admissions List daily. Complete short term admissions paperwork with Medicare admissions within 48 hours of admission. Assist with gathering documents for Managed Care updates. Weekly Therapy Care Plan Updates. Stock rooms with Gait Belts. Update white boards.
6. Enters and/or scans charge tickets from supply room charge book upon discharge of resident or at end of month. 20%
7. Cross train within department to cover team members as assigned by supervisor. 3%
8. Performs other work duties as assigned. 2%

Education
High school diploma or GED required.
Experience
35-40 wpm typing and basic computer skills
Clerical and numerical ability are necessary
Verbal and cognitive ability are necessary
The assessment, care and treatment provided by the staff will be consistent with the specific age related needs of the patient. The staff is competent to care for patient age groups including the adult and geriatric patient.
