Description

The Supervisor, Care Management leads teams of health professionals responsible for assessing and evaluating patient's needs and requirements to achieve and/or maintain optimal wellness state by guiding patients/families toward and facilitate interaction with resources appropriate for the care and wellbeing of members. The Supervisor, Care Management works within thorough, prescribed guidelines and procedures; uses independent judgment requiring analysis of variable factors to solve basic problems; collaborates with management and top professionals/specialists in selection of methods, techniques, and analytical approach.

Responsibilities

The Supervisor, Care Management leads a team of non-clinical health professionals responsible for assessing and evaluating patients' needs and requirements for prescription assistance program and/or other social . The prescription assistance program (PCP) team guides patients/families towards and facilitates interactions with appropriate resources working across the CenterWell and Conviva markets. Decisions are typically related to schedule, plans and daily operations. Performs escalated or more complex work of a similar nature, and supervises, coordinates and provides day-to-day oversight to associates. Ensures consistency in execution across the team and markets. Holds team members accountable for following established policies. Additional responsibilities include participating, facilitating, or leading workgroups, training new hires, and developing training or program content.

Required Qualifications

Healthcare experience

3 or more years of professional experience

2 years prior supervisory experience with ability to provide mentorship and coaching as needed

Proven ability to work independently and demonstrate critical thinking skills

Exceptional communication and interpersonal skills

Team-focused

Able to demonstrate proficiency with Microsoft Office Products

Preferred Qualifications

Social work experience in addressing social determinants of health needs

Health plan experience

Experience with program implementation

Prior Medicare / Medicaid experience

Additional Information

For this job, associates are required to be fully COVID vaccinated or undergo weekly COVID testing and wear a face covering while at work. The weekly testing will need to be done through an approved Humana vendor, and unvaccinated associates should follow all social distancing and masking protocols if they are required to come into a Humana facility or work outside of their home.
