\u003cp\u003eThe Telecare Shelton Evaluation and Treatment (E\u0026amp;T) center will provide intensive mental health and psychiatric treatment services in a secure, welcoming and respectful environment for adults experiencing an acute mental health emergency. \u003cp\u003e\u003cstrong\u003ePOSITION SUMMARY\u003c/strong\u003e\u003cbr /\u003eThe Peer Health Navigator engages and connects members served to mental health, primary care, substance use, and specialty health care services; helps them develop personal health and wellness goals; educates them on how to advocate for themselves and effectively communicate their needs; and supports them in overcoming barriers and building resilience. The Peer Health Navigator supports members served in maintaining a follow-up care plan and developing other self-management skills through a \u0026ldquo;Modeling \u0026ndash; navigator performs task; member served observes, Coaching \u0026ndash; member served performs task; navigator coaches, Fading \u0026ndash; member served self-directs and manages own healthcare; navigator assist as needed\u0026rdquo; process. The Peer Health Navigator simultaneously serves as a role model of resilience, recovery, and self-responsibility for their own mental and physical health. They continually promote and reinforce Telecare\u0026rsquo;s Recovery Culture as defined by the Telecare Recovery Centered Clinical System (RCCS) in which the member\u0026rsquo;s point of view and preferences are recognized, understood, respected and integrated into services and self-help programming. Additionally, Peer Health Navigators will respond to critical situations with high-level engagement and de-escalation skills which support a least restrictive environment for members served experiencing an emergency related to a mental health/addiction challenge.\u003c/p\u003e
\u003cp\u003e\u003cstrong\u003eESSENTIAL FUNCTIONS\u003c/strong\u003e\u003cbr /\u003e\u0026bull; Demonstrates the Telecare mission, purpose, values and beliefs in everyday language and contact with the internal and external stakeholders\u003cbr /\u003e\u0026bull; Draws upon own personal recovery experience with mental health/addiction challenges as a basis for engagement as a peer with the members served and to validate members\u0026rsquo; own lived experiences\u003cbr /\u003e\u0026bull; Assists members served with voicing their strengths, interests, and goals throughout their time in services including: goal plans, crisis plans and other methods of expressing individual preferences\u003cbr /\u003e\u0026bull; Utilizes a comprehensive resource guide and educates members about local community supports and resources\u003cbr /\u003e\u0026bull; Supports members served in developing strategies to reduce self-stigma\u003cbr /\u003e\u0026bull; Engages and communicates with a wide range of members served especially those who are struggling to be active in their own recovery\u003cbr /\u003e\u0026bull; Assists participants in all needs including Activities of Daily Living (ADLs)\u003cbr /\u003e\u0026bull; Actively participates in multidisciplinary team meetings\u003cbr /\u003e\u0026bull; Participates in verbal de-escalation and physical interventions in emergent situations and is willing and able to assist other staff as needed to maintain a safe, healing environment for all\u003cbr /\u003e\u0026bull; Travels locally to provide outreach to members served in the community (program specific)\u003cbr /\u003e\u0026bull; Must assist with restraint of members served in the event of assaultive behavior and pass assault crisis/crisis prevention training\u003cbr /\u003eDuties and responsibilities may be added, deleted and/or changed at the discretion of management.\u003c/p\u003e
