Coordinates all pre- and
post-transplant care. Assists other
health care personnel in identifying health care needs. Interacts with patients and families and all
levels of personnel, medical staff, community services, and insurance
providers. May provide care to persons
of diverse ages and cultures. Identifies
and coordinates educational programs for patients and health care
professionals, as well as the community.
Interprets standards for the care of a specific patient population and
provides clinical, consultative, educational and research support to ensure
that quality cost-effective care is consistently provided. Communicates across health care areas to
ensure that systems integrate efficiently in order to best serve patients,
staff, physicians and community.
ESSENTIAL FUNCTIONS:

Acts as a liaison among the individual/family,
community, provider and physician as well as other health care team members.
·
Consults with interdisciplinary teams in the
delivery of client/community care necessary to meet identified/negotiated goals
across all health care settings.
·
Collaborates in developing, implementing and
evaluating teaching/learning strategies for client/family.
·
Coordinates appropriate educational
material/instructions for patients, families, medical staff and
multidisciplinary personnel to accomplish patient care outcomes.
·
Establishes and coordinates patient/community
rounds, as appropriate.
·
Identifies specific needs for continuing
education based on outcomes.
·
Participates in planning and presentation of
continuing education programs, seminars and meetings.
·
Utilizes expert knowledge and teamwork to
facilitate cohesive efforts to achieve optimal outcomes in providing for
patient/populations and/or communities.
·
Assists with the development, implementation and
evaluation of clinical tools that targets time frames for expected outcome
events determined through collaboration with health care team members.
·
Coordinates with the health care team to develop
a mechanism to monitor patient outcomes and follows up on variances.
·
Ensures collection and input of risk
stratification data. Reports results to
the health care team and participates in interpretation of data and formulation
of action plan.
·
Acts as a steward of resources in providing
prudent health care services.
·
Serves on selected hospital, group practice and
community-based committees.
·
Identifies need for and participates in research
projects relevant to specific patient population.','!*!MINIMUM KNOWLEDGE, SKILLS AND ABILITIES REQUIRED:
·
Possession of a current or compact state license
as a Registered Nurse issued/defined by the State of Iowa.
·
BSN Preferred.
·
Must become NATC certified within 5 years.
·
Must attend a transplant related conference
every 3-4 years.
·
Proof of
completion of Mandatory Reporter – Child and/or Dependent Adult Abuse training
within three (3) months of hire.
·
Basic Life Support certified within six (6)
weeks of hire. Acceptable credentialing
bodies and certifications include American Red Cross CPR/AED for the
Professional Rescuer and American Heart Association Basic Life Support for
