SUMMARY:
This position
is responsible for coordinating Quality, Risk and Safety improvement
initiatives as assigned by the Quality Director for the assigned KentuckyOne
entity. Responsibilities include
facilitation of improvement teams, planning, implementing and monitoring improvement
strategies, using sound performance improvement methodologies, educating
hospital and medical staff on performance improvement initiatives, assisting
with data collection, aggregation, analysis and interpretation, and preparing
statistical and analytical reports to support the organization’s Quality and
Safety Plan.
PURPOSE:
To support
the organization’s efforts to implement and evaluate the Quality and Safety
Plan.
Essential Functions
• Under the direction of the Quality director, concurrently
monitors documentation of care provided to patients included in quality based
payment reform programs (value based purchasing/core measures,
hospital-acquired conditions reduction program and hospital readmission
reduction).
• Provides timely feedback to clinical and medical staff to
facilitate accurate data capture of patient care provided in the electronic
medical record.
• Provides timely education for clinical and medical staff
regarding changes quality based payment reform documentation requirements.
• Performs data collection, aggregation and analysis to
support external data reporting requirements.
• Collaborates with clinical and medical staff leaders to
develop, implement and evaluate process improvements to improve organizational
performance and patient outcomes.
• Performs data abstraction, validation and submission
processes as assigned to meet externally reported data requirements, including
quality-based payment reform programs, registries, third-party payor quality
programs, and KYOne, CHI, and national quality initiatives.
• Serves as a member of the SafetyFirst Core Team to review
and classify IRIS reports utilizing the Healthcare Performance Improvement
(HPI) classification system.
• Participates in root cause and common cause analyses as
assigned.
• Serves on performance improvement and patient safety
committees or teams as assigned.
• Assists with efforts to ensure continuous organizational
compliance with regulatory requirements and accreditation standards.
• Performs related duties as required.

Accreditation / Licensure (required & preferred):
• Bachelor’s degree in health administration, nursing,
business administration or related field.
Current, valid Kentucky license required for registered nurses.
• Experience (required and preferred):
• A minimum of five years of progressively responsible
work-related experience in hospitals/health care. Additional qualifying work experience may be
substituted for the degree requirement. Previous experience in performance
improvement a healthcare setting is preferred.
